It is important to first establish your value within the company in your performance review. You should be able to list the high points of what you've been able to accomplish.
When discussing a raise it is always good to start with gratitude and appreciation. Recognize what the company has done for you and acknowledge that.
Be prepared to give specific examples of what you have contributed to deserve the raise and discuss your plans for the future.
Be sure to follow up with a thank-you note or email for the time and attention your employer devoted to discussing your development opportunities even if you don't receive the raise.
